ABC转运蛋白超家族与乳腺癌多药耐药论文_黄滨,雷小勇(通讯作者)

摘要:乳腺癌的发病率和死亡率均为女性恶性肿瘤的首位。化疗在乳腺癌综合治疗中占有极其重要的地位,而多药耐药(MDR)的产生是影响乳腺癌患者化疗疗效及存活率的主要原因,其产生机制错综复杂。ABC转运蛋白超家族可以调控胞内药物的积聚和分布,从而诱发耐药。本文将对近年来ABC转运蛋白超家族与乳腺癌多药耐药之间的进展予以综述。

据统计,乳腺癌的发病率和死亡率均为女性恶性肿瘤的首位[1]。化疗是乳腺癌综合治疗中的重要手段,能明显降低乳腺癌的术后复发率。尽管早期乳腺癌的5年生存率较高,晚期乳腺癌及三阴性乳腺癌的预后却较差,多药耐药(multi-drug resistance,MDR)是乳腺癌化疗失败的主要原因之一。

多药耐药是指肿瘤细胞接触一种抗癌药物产生耐药,同时对其他多种结构和作用机制完全不同的抗癌药物产生交叉耐药的一种独特现象。多药耐药的产生机制包括药物作用靶点的改变、细胞凋亡抑制和细胞周期停滞、DNA甲基化等表观遗传学改变、腺苷三磷酸结合盒(ATP-binding cassette,ABC)转运蛋白超家族的异常表达等等[2]。ABC转运蛋白超家族是跨膜转运蛋白中一类特殊的蛋白,其在多药耐药的产生中占有重要的地位。本文将对ABC转运蛋白家族与乳腺癌多药耐药之间的关系进行综述。

1.ABC转运蛋白超家族

ABC转运蛋白超家族是人类最大的跨膜转运蛋白,目前在人类基因组已知的成员有48个,被分为7个亚家族(ABCA-ABCG)。作为细胞膜上的跨膜蛋白,ABC转运蛋白是依赖ATP能量的单向底物外排泵,以主动转运的方式完成多种分子的跨膜转运,其底物包括氨基酸、胆固醇及其衍生物、糖类、维生素、肽类、脂质、一些重要的蛋白质、疏水性药物和抗生素等。同样,ABC转运蛋白通过利用ATP水解成ADP释放的能量将细胞内的药物逆浓度泵出细胞,降低细胞内的药物浓度,从而使肿瘤细胞对多种化疗药物产生耐药。
